Lease Affidavit Form????

I live in Texas and I'm hoping someone in this forum leases an Infiniti in Texas also. I am trying to get a fresh copy of the lease affidavit form that is sent out each year. This is the form that you fill out to be exempt from yearly property taxes. If you have it please message me or email me a copy to stang18rmj at yahoo.com
