Symptoms... Bluetooth will not connect. All phones deleted from radio. When going to add device, Bluetooth icon in radio continues flashing.

Is the Bluetooth module separate from the radio unit? I see some people mention behind the glove box, others behind the radio.

Second, is there a way to reset the Bluetooth module? Pulling a fuse or otherwise? I disconnected the battery for 15 minutes and that didn't seem to reset the radio.
 
The bluetooth module is separate from the radio. So if you're standing at the back of the car with the hatch open, it's on the inside right corner near the floor. It's a module basically and there's cables running to it. To access it you'll need to take the floor that lifts, subwoofer, spare tire, tether hooks, the plastic that surrounds the spare out. You'll find the module under there.

What's the story on it? Did it work at some point and suddenly stopped?

I'd start with getting access to that module and inspecting it. Remove all the connections, check if there may have been water intrusion of some kind or maybe corrosion on the connectors. There's what appears to be a cable that connects to it and acts like an antenna.

When taking the interior paneling near the spare off, take care with removing all the bolts/screws. There's one closer to the rear seats in the middle. There's also an antenna there that needs unclipping. The tether hooks near the rear seats on the floor need care when unclipping the cap to access the 10mm bolt. I used a small flat screwdriver to push the tabs inwards and up. Don't break those, they're not cheap!

Once you disconnect and reconnect, give it another try and see if anything changes.
