So every time my 2005 FX35 with 145k miles is started after setting for a few hours the first 2 miles or so has a higher rev pitch.
I do not remember this from before but I may be wrong and just looking for issues.

Help me keep my ageing beauty on the road

I would open the hood and try to identify where the noise is coming from while the noise is screaming. It might be the belt or one of the idlers?

On your other post you said you have warning lights on and that you found one of your brake pads is worn out plus a sticking calliper. This high pitched metallic noise could be your worn out pads rubbing on your rotors.

It almost sounds like a supercharger.

I assume a sticking Caliper haven't pulled again to test it. Was gonna open it up and rev in neutral see if same thing. I heard a similar story on click and clack and it was a sticking pulley or an exhaust leak. If I remember right.

^if it's something like that, it might be the heat shields, if you still have the stock exhaust

1 1/2 years later...Ever figure it out? I have this issue as well at start up. It does sound like a supercharger. Guess I don't need one now. Like'n the sound. Didn't notice it until I deleted idler pulley & heater hoses. I think (think, don't know) it's the power steering pump.
